The following forms need to be completed by the mother/legal guardian and returned to Guys Like Us, PO Box 178, Hinsdale, IL 60522-0178 in order to register for Guys Like Us. Please note that all information will be kept confidential.

The Program Registration Form provides contact and background information on the family and your son which is the most critical form to be completed. Please spend time completing this form and providing all necessary information.

The Health History Form provides Guys Like Us with information about the overall health of your son and will only be used in case of emergency, but still needs to be completed.

The Indemnification Agreement is a non-negotiable form that is required for your son’s participation in our program.

Also, the Program Registration Form (b) should be completed by your son and returned with the other registration materials.

Role of a Mentor which provides a description of the mentor’s role in the lifestyle of your son.

Guidelines for Choosing a Mentor provide guidelines from Guys Like Us when thinking about choosing the perfect fit to serve as your son’s mentor while in the program.
